Position Summary

As a Psychiatric Case Manager, you will play a pivotal role in the holistic care and support of individuals facing mental health challenges. Your primary responsibility will be to assess, plan, coordinate, and monitor the services and resources necessary for our patients' mental health treatment and overall well-being. You will work closely with interdisciplinary teams to develop and implement comprehensive care plans tailored to each patient's unique needs.

Responsibilities:

 

  •  Act as a strong advocate for patients, ensuring their needs are met, their rights are protected, and they receive the support necessary for their mental health journey.

  • Coordinate and monitor patient care with healthcare professionals, including psychiatrists, therapists, and primary care providers, ensuring integrated and holistic patient support.

  • Provide patients and their families with education on mental health issues, treatment options, and available community resources, empowering them to make informed decisions.

  • Monitor and record patient progress, adjusting care plans as necessary to ensure their mental health needs are met effectively.

  • Connect patients with relevant community resources, such as support groups and social services, facilitating their access to additional support.

  • Maintain accurate and up-to-date patient records, ensuring compliance with healthcare regulations and privacy standards.

 

Qualifications:

 

  • A caring and empathetic approach when dealing with patients, understanding the challenges they face and providing support with patience and understanding.

  • Strong communication skills, both written and verbal, to convey information clearly and supportively to patients and their families.

  • Ability to assess patient needs, find suitable solutions, and collaborate effectively with healthcare professionals and external resources.

  • Willingness to adapt to different situations and patient needs, showing flexibility in managing diverse cases.

  • Comfortable using telemedicine platforms and electronic record-keeping systems.

  • Previous experience in case management or a related field is beneficial but not required. A strong desire to help others and a positive attitude are equally important.
